Description

The Adult/Adolescent Sensory History Scoring Program© is designed to score the Self-Report Questionnaire and Caregiver Questionnaire. The test administrator enters demographic information as well as item responses into either a fillable PDF, or excel spreadsheet. A one-page report form, accounting for missing items, is generated automatically. The report provides raw scores for all test subscores as well as score interpretations. With the PDF version the test administrator inputs z or standard scores using the look up tables provided, the excel spreadsheet automatically provides z or standard scores. The single page report form is suitable for storage in client records, and may be printed to give to clients

Requires Adobe Reader (free) on a PC or Mac, or Microsoft Excel on a PC with macros enabled.

One license provides unlimited use, when used with the Self-Report or Caregiver Questionnaire, for up to 30 users.
